It's clear that engaging citizens in their own health is a must-do if we expect better outcomes at lower costs. Making that happen at scale has been challenging -- but a recent set of public/private initatives may be changing that. Programs like the Direct Project, Blue Button, Meaningful Use and the Health Data Initiative are getting more data into the hands of citizens, and tools for self-managed and collaborative care are emerging to take advantage of these new data streams.
