No preflight would be correct. 100% pilot error. A pilot gets in a hurry and not paying attention. A 10 second control check would have saved all 3 lives. Never be in such a hurry to fly that you skip your walk around and preflight.

Do a little research before speculating. They forgot to remove the control locks, this is what caused the excessive nose up attitude resulting in a spin and eventual crash.
