This 9 minute video shows the treatment of a patient with a fractured, root canal treated tooth by the preparation and fabrication of a Flexi Post and core/crown buildup (used to re-enforce the tooth and provide a 3 dimensional template for the crown) which was followed immediately by preparation, imaging, design, milling, polishing and cementing a CEREC all-porcelain crown in under two hours. The post and core procedure steps included removal of fractured tooth, remaining caries (decay) and old amalgam and temporary filling, which was placed after the root canal treatment, and preparation of the post-hole and fitting the FlexiPost #1 The post was evaluated with Schick CDR digital x-ray confirmation (in 7 seconds after exposure) and we then see the cementing of the FlexiPost with dentin bonding using Amalgambond, FlexiFlow Cementing of the post and a TiCore Buildup in an AutoMatrix. The crown buildup was adjusted for ideal occlusion (bite and excursions) and the CEREC optical impression was made to correlate with the crown preparation that followed. The CEREC BluCam then captured the preparation and the crown was designed on the Acquisition Unit with the patient watching over Dr Rosenberg's shoulder. A dental assistant measured the color of the adjacent teeth with the Vita EasyShade spectrophotometer and a corresponding block of Vita porcelain was selected to match the shade of the adjacent teeth. The computer in the Acquisition Unit then directed the MCXL milling unit digitally to mill the selected porcelain block into a crown that precisely fits the crown preparation that was acquired with the BluCam (blue laser 3-D camera). The patient was amazed to watch the block of porcelain milled into her crown. The milled crown emerged from the milling unit in under 10 minutes, the contacts of the crown with the adjacent teeth were then adjusted and the porcelain crown was polished to a high lustrous shine. The inside of the crown was etched with hydrofluoric acid and a silane coupling agent was applied inside the etched crown. The prepared tooth was etched and Excite bonding agent was applied and thinned. The crown was cemented with Ivoclar Multilink Cement and light cured. The occlusion was checked and adjusted slightly and a final repolishing of any areas that were adjusted was accomplished. The patient who is the mother of an existing adult patient in the practice was amazed at the technology used to restore her fractured, root-canal treated tooth in under 2 hours from entering the office.

Every Cerec crown I have seen has been on the end of an extracted tooth! I dont use Cerec because they fail due to poor marginal fit. For me, I ask the patient how long do you want it to last? If they answer forever, then it s gold and if its in the smile line or they want an aesthetic crown then its an emax made by a dental tech.
