In my opinion, they eased the difficulty on this GUILT compared to UtK1. Pempti will always and forever throw out 6 of any core (laceration, tumor, or revolving). It's more helpful to know this for the laceration cores since you can use the stabilizer or suture something after the sixth one.

I leave two tumor cores there for the 130 chain bonus.

I focus a lot on the bottom main core since it has 6 HP, compared to the top's 4 HP.

Funny story - I just did it for the first time after 8 hours (on and off) of solid pummeling via Pempti, I finally do it, and I'm too busy jumping around and fist-pumping to notice that I'd left two cuts and a few polyp holes on the lung, and before I had a chance to heal them up and pump vitals, the patient was dead. BAH!!!!!
