Correction on the previous comment: a neutral position of the shoulder would be with the palm forward, as in anatomical position. A thumb up position would be in internal rotation.

correction: if pain, it suggests tendinitis of the supraspinatus, not of the long head of the biceps. Great video anyway!

@bigesor thumb down is internal rotation, and yes for jobe test.
@bigesor thumb-up is external rotation not internal!!! remember the jobe-test.
