Just to answer something, the Lifespan of a "Smart" AI, an AI like Cortana, has a lifespan of about 7 years, which will make her appearence older. She might die during Halo 4 from age (Incapacity, as she is a computer program), due to the span between Halo 3 and Halo 4 being unknown. Cortana is one of the older "Smart" AI's, and if Halo 4 is 2-3 years after Halo 3, she's about to go dark. As far as the planet, it's probably Forerunner. Maybe we get to dwell into the past of Halo history? I hope.
