Myomectomy inferior oblique with Fugo blade

The lower fornix is opened with Fugo blade, lateral to the inferior rectus muscle. The fleshy inferior oblique is reached after going through the thick capsule, catching the fleshy muscle, passing a muscle hook underneath it, making sure the fully belly is over the hook.After this the exposed muscle is incised with Fugo blade, without bleeding and without burning.The conjunctiva is closed with a couple of sutures. A job of a few minutes only.

I do not save the strip any more.I have improved the technique further. I go straight to the muscle, which is about 14 mm from the limbus. The inferior oblique is cut within a few minutes.

Hi, very educational. What is the reason for leaving a small strip of the muscle? is there a different outcome when compared to a complete separation?
