@shaun5515 That is indeed the big question in all SH games and Jacobs Ladder. The movie and most of the games strongly imply that the main character is in some kind of hell or purgatory after he died. It is never made 100% clear, but it is implied. I think that is the real horror about it, never knowing if it is reality, a dream, or a horrible mix of them.
