Paula L. Stillman, M.D., MBA, MACP, Professor of Medicine & Pediatrics, health care consultant and former Vice President of Health Care Services for Temple University Health System, presented "Population Health: What Does This Mean?" at University of Louisville Department of Medicine Grand Rounds on March 20, 2014.

The talk focused on developing understanding of key concepts of population health and how to stratify providers, sites of care and patients, becoming familiar with community health workers and how they can function as members of the care management team, and understanding trajectory from fee for service to total capitation and where we are on the continuum.
