John Newton, who is best known for having written "Amazing Grace", wrote "Savior, Visit Thy Plantation", which appeared in Newton's Olney Hymns, first published in 1779.
Olney Hymns can be found online at: http://www.ccel.org/ccel/newton/olneyhymns.html
Saviour, Visit Thy Plantation is shown at: http://www.ccel.org/ccel/newton/olneyhymns.h2_51.html
Over time, the hymn found its way to North America and was converted into a shape note song, and had the "Return Again" chorus added. In this form, it appeared in "The Sacred Harp". The shape note song "Return Again" can be found at: http://digital.lib.msu.edu/projects/ssb/display.cfm?TitleID=610&Format=jp... (not a good reproduction, however).
I have taken the shape note version and added more verses (from Olney Hymns) to flesh out the version presented here.

Many thanks for PLBrayfield.
Very well done! I have only ever sung this in the 4-part Sacred Harp harmony, and hearing just the melody line with guitar chords makes it fresh and new to my ear.
