Sounds like unfit-to-string hammers and new hammers on old shanks with hammer butt flanges very sloppy.

Ha Ha - this is a poor old practice room upright at the local school. Only funds for tuning once or so a year. Lots of cracks, crazy humidity and temperature swings makes for a tough environment...
