An older gentleman (95) in the crowd gives Obama a stick to which Obama replies he would use on Congress if they don't pass his health care bill.

He was not being literal or symbolic. He was just joking around and being very kind to a nice old man. Obama has a sense of humor, so lighten up!!

I would suggest that you have no more idea of what he was doing than I do.
If you ever watch State of the Union speeches, you might have noticed that every President for at least 40 years has guests in the balcony that they use as influence for their agenda.
