It's another day on the road for Sam, Dean and the Impala. Today, they're in Midland, Texas, in an old diner with a young waitress named Sandra. As usual, Dean flirts, but they actually hit it off, Sandra falling for free-spirited Dean, and their tales of life on the road.
A few days later, Dean notices that Sandra is acting strange. She avoids Dean, and Dean concludes that she's scared of something. Grimly, Sandra explains that "All she wants is a normal life", surprising Dean, then proceeding to tell him her story. She explains a troubled father, a mother who left because she couldn't take it anymore, and her ending up leaving school. When Dean sees how angry she is, he voices his disapproval to her. Sandra apologises for dumping everything on him.
As days go by, Sandra becomes infatuated with Dean and Sam's freedom, and expresses how she would love to join them. Dean thinks it is a joke, but she packs her money and a credit card, and boldly asks Dean to elope with her. She imagines life on the road and recalls memories of love and security, hoping to find it with Dean. Dean is taken aback, suspecting someone has put her up to it, but she simply puts that she "has her reasons" and that she promises to "tell him everything", baffling Dean.
That night, on the way back from their hunt, Dean thinks about Sandra. He goes to see her the next day, and she asks him again if he will take her with them. Unable to deny her, he agrees, and kiss, despite feeling something is off. Sandra is estatic, but once outside inside his car, while waiting for Sam, Dean thinks carefully about his choice. Realising that he's making a mistake, he starts up the car with Sam inside, Sandra watching from the diner, shocked.
One week later....
After a hunt, Sam and Dean are at Bobby's recuperating. As Dean works on a car, Sam tries to ask how he's doing, noticing something is wrong. Dean brushes him off, so instead, Sam gives Dean a listen to a strange phone call Bobby passed to them. It's a possible case, and Sam's already tracked down an address, so they investigate. They end up with an old house in the suburbs, and sneak in.
As they snoop around, Dean finds the pictures in the frames eeriely familiar to him. He gets freaked out, until he comes upon one picture that shocks him to the bone, prompting him to leave immediately. Following Dean's lead, they end up in New York, where Dean goes in to find, as he expected, Sandra.
He understands now why he felt that something was off, but Sandra refuses to listen to him, still furious at being left behind. Dean explains that he had wanted to stay, but Sandra doesn't care. He explains things to Sandra, about he and Sam killing supernatural beings, and about the case from Bobby. Sandra is freaked out by the explanation, and tries to run away. As she runs, Dean's struggles with killing her or not, as memories flash through his mind. Then finally, with tears in his eyes, he shoots her. He now understands why she wanted all those things from him.
When Sam hears about what Dean has done, he confronts him and demands to know why he would shoot a young woman, who as far as Sam knew, was guilty of nothing. Dean argues that she was a supernatural thing, and no matter what they're obligated to kill it. Sam argues that they only kill evil, to which Dean simply puts that they didn't even know her anyway. Infuriated, Sam fires back that Dean's lying, because he knew who that girl was, that she was the waitress from Midland, and that he knew Dean loved her: that was why he let Dean go in alone. Struck, Dean takes it in, then punches Sam.
Alone and feeling guilty, Dean remembers Sandra before Midland. Back when she was Claire Bennet, a young, blonde cheerleader, whom he'd known from afar. On homecoming night he had bumped into her, the only time they ever met, before he left Odessa, and she went about her business. Sandra was supernatural, because Claire Bennet was killed on Homecoming night. Dean never got over the guilt.
Back in the hotel room, Sam tells Dean that the hospital managed to stabilize Sandra. Dean shrugs, until Sam presses on and Dean explains everything. Dean voices how the amount of loss and sacrifices they've made was just too much. Sam comforts him by saying that their decisions save lives, and it's worth it all. Realising that there is nothing he can do to change it, Dean and Sam pack up for the road.
Then with one last look behind, Dean gets in his car, and drives away.
