From 1996-2006, the guiding principle before immunisation used to be that a child's fitness and suitability had to be established. Refer to the "Green Book" crown copyright. A child's fitness and suitability is no longer mentioned.

Dear Ms Filby,




Thank you for your email of 27 September about the Consent chapter of the 'Immunisation against infectious disease' (the Green Book). I have been asked to reply.

I note your concern that the guiding principle of a child's fitness and suitability prior to vaccination, which is discussed in the Consent chapter of the 1996 edition of the Green Book, no longer appears in the Consent chapter of the 2006 publication. The Department of Health would suggest that, despite the exact wording 'fitness and suitability' not being present in the new Consent chapter, the guiding principle is dealt with throughout the chapter, particularly on pages seven and eight, as well as in other sections of the Green Book.

For example, the following guidance is given in the Consent chapter, on page eight:

'health professionals should ensure that the individual (or those giving consent on their behalf) fully understands which immunisation(s) are to be administered; the disease(s) against which they will protect; the risks of not proceeding; the side effects that may occur and how these should be dealt with; and any follow-up action required.'

Furthermore, the following guidance is given in Chapter, Immunisation Procedures, on page 26:

'Before any vaccine is given, consent must be obtained (see Chapter 2) and suitability for immunisation must be established with the individual to be vaccinated, or their parent or carer.'

It is also important to note that chapters 13-35 of the Green Book deal with individual diseases, their vaccines and the process by which the vaccines should be administered to patients. This includes the discussion of recommendations for the use of the vaccine and possible adverse reactions and contraindications of each vaccine. This information provides sufficient guidance on the guiding principles surrounding vaccinations.

To ensure that patients fully understand their immunisations, health professionals supply patients with information materials prior to immunisation. These materials also outline the guiding principle of fitness and suitability prior to vaccination.

For example, you may wish to view the Department's publication, 'A Guide to immunisations up to 13 months of age'. Pages 13 and 14 describe in detail the state of health patients should be in prior to vaccination. This leaflet is available from the Department's website by clicking on the following link:
