The cable leg extension performed free standing develops leg (quadriceps), glute, and core strength. This is an excellent exercise for runners because it mimics the drive phase of the run. Keeping your hands unanchored requires the core muscles and hips to help stabilize the body during the exercise.
There is no reason not to hold onto anything, unless you are training specifically for balance or general functionality (and let's admit it, *most* people at the gym are not training for functionality, they are training for muscle growth).
Hold onto the rail in front, which will increase stability and allow a greater amount of weight to be lifted, thereby increasing strength gains.
