@99percentatheist well its kind of like 1999 mode in bioshock infinite. It's only avaliable after you beat the game once and then you have the option of doing it. Basically, enemies are harder, those flower heart things almost never show up (and really almost all hearts for that matter, and I also think the stamina gauge decreases even more rapidly. And perhaps there aren't any stamina fruits? Either way it's more difficult.

I've played almost all Zelda games except for the one's made by Capcom and I got to say this one is probably the easiest one of them all. They really dumbed this one down.
