if you look for a PCB make sure it's for 1988 Sega Tetris and not 1999 :)

you need a playstation 2 (must be Asian model) then,
google search:
sega ages vol. 28
(it includes the original Sega Tetris)
or you could try to find the Sega Tetris PCB and build an arcade machine, or maybe you'll find the original Sega Tetris machine somewhere.
