To achieve the desired refractive outcome following cataract surgery, many options exist and an IOL exchange is a reasonable choice in many situations. This patient is 2 and 1/2 months post-op from the original cataract surgery, and despite multiple pre-op measurements and calculations the refractive result was myopic. Given the options, the patient chose IOL exchange. Wound placement, viscodissection, trisection of the IOL and efforts to ensure complete removal of the lens are all important elements to achieving a desired result.
