Ideally we need a retentive viscoelastic to coat the cornea during phacoemulsification. After implanting the IOL we need a visco that will leave the eye as completely as possible to prevent post operative pressure spikes. Initially we need a dispersive viscoelastic like methycellulose and finally a cohesive viscoelastic like Sodium Hyaluronate. A mixture is not the best option, Zeiss company's twin visc is the most logical option. other brands that are a combination of sodium hyaluronte and chondroitin sulphate in various concentrations are at best a compromise. Ideally we need to use methylcellulose first, to protect the cornea during phaco . After IA we need to inject sodium hyaluronate to hold the chamber during IOL insertion.
