My Discus Planted tank

My 50 gallon planted tank. Pardon the white particles floating around, they are bits of the erythromycin tablets that i dosed to remove blue-green algae that is infesting my dwarf hairgrass.
Flora : eleocharis parvula, blyxia japonica, ludwigia var cuba, heteranthera zosterifolia, rotala walichi, rotala sp green, eustralis stellata, rotala macrandra, hygrophilia corymbosa angustifolia, camboba furcata, limnophilia aromatica and fontinalis antipyretica
Fauna: 3x juli corydora, 4x siamese algae eater, 4x otocinclus affinis, 7 neon tetra, 6 rummynose tetra, 5 discus
Lighting: 4 x 21 watts T5 + analog timer
Filtration: Eheim liberty 200
Co2: 5lb tank + dual stage regulator + co2 reactor
Fertilization: Hagen PlantGro + UP Aqua Ferro nutrients
Music: Barcelona - Please don't go

its fantastic wow. My tank was nice to but like last 2 months all my plants starting to die out? idk why.. i use CO2 but they all kind of die slowly. any idea? do they need some kind of minerals?
