Natural Orifice Transluminal Endoscopic Surgery is a novel, emerging section of modern surgery with a great potential. Its therefore no wonder that the related research has become of great interest worldwide. Our aim, as the NOTES workgroup at the Department of Surgical Research and Techniques, University of Pécs, Hungary, is to develop the method and to introduce NOTES to the clinical routine.

The first NOTES surgery in Hungary carried out in our institute; October 2008 was a transvaginal hybrid cholecystectomy, done in a porcine model. During the intervention we used rigid surgical tools with a hybrid technique. Namely, the preparation of the bladder and the placement of the clips were carried out through a trochar placed 5 cm above the navel, but the removal of the gallbladder was done transvaginally. We followed this successfully first surgery with five further operations, in which we also tested for liver enzymes and for oxidative stress marker activity.

In the future we plan to do some refinement of surgical tools and complete the development of a so called overtube, which functions as a working channel during transvaginal surgery and also protects the visceral organs. Furthermore we wish to find a solution for intra abdominal illumination and camera actuation by applying methods from robotic surgery.
