but the resulting jump is anything but smooth.

Lol @ all the people bashing the instructor. 99% of people who go skydiving get cold feet and refuse at the door. It's pretty much universal policy to push them out the door. It was the old ladies fault for what happened, she was not in the correct jumping position which is why she slipped out of the harness. One loose shoulder strap would have been fine, it was because she crouched down and was in such a position when they jumped she fell through. It's nobody's fault by hers. Deal with it.

She should sue. Firstly, she obviously wasn't wearing the right gear. Second, she obviously wasn't safely strapped in. Finally, you could see she had clearly changed her mind and he just grabbed her and dived out. They could've killed her and she's so lucky she didn't have a heart attack. Horrible to watch!
