Try standing closer to the ball. You are reaching a lot which causes your swing plane to be too flat (rounded) which will cause all kinds of problems--both slices and hooks.
Stand comfortably with your arms hanging. It will feel very weird at first and you might feel like your swing is too upright or you're going to shank every shot but with some practice you'll start hitting much straighter, pure shots with a more upright swing.
