Starting from low E showing the frets where his fingers are I think it's:

I think his guitar is tuned half a step down.

Funny, for some reason, I always go to these videos thinking I'm going to hear complex and beautiful sounding chords. I forget it's usually someone doing a simple lead on one or two strings with sustain and a phaser. Granted the last chord was pretty.
