#1: The Eagle Newspaper
Act I Chapter 1:
Down the road a little bit as you start the training mission.

#2: Ambulance Driver's Log
Act I Chapter 1:
At the end of training, go down the path in the opposite direction of the hospital.

#3: Doctor's Journal
Act I Chapter 2:
Near the beginning of the chapter, go left through a door down the hospital hallway.

#4: Jacinto Medical Center File
Act I Chapter 2:
In between two reception desks where you fight some locusts.

#5: Cog Letterhead
Act I Chapter 2:
In a small room across from a kitchen.

#6: Cog Proclamation
Act I Chapter 2:
Turn right at the top of some stairs, to find it at the end of a dark balcony.

#7: Cog Tag
Act I Chapter 2:
When you first exit the building, turn right to find a cog tag in a corner.

#8: Grindlift Notice
Act I Chapter 3:
At the very beginning of the chapter, on a wall of the vehicle you're riding.

#9: Munitions Requisition Form
Act I Chapter 3:
When you're first on the ground again, go left to find it by some ammo crates.

#10: Jacinto Sentinel Newspaper
Act I Chapter 5:
Through the door at the beginning, hop the counter, and go up the stairs.

#11: Memorial Inscription
Act I Chapter 5:
Down the street before you get to the gas station, turn right at the intersection.

#12: Truck Driver's Note
Act I Chapter 5:
Inside a small room in the gas station.

#13: Cog Tag
Act I Chapter 5:
Through the dark tunnel, go left into a doorway before a set of stairs.

Play social, it has a map cycle and just play and stay in the match, eventually you will play on that map.

does anyone know a easy way to do the 'annex now with execution rules' achievement, all i need to do is Gold Rush, but the map never comes up !
