I performed a Big Bubble Technique and a Phacoemulsification with IOL Implantation in a patient with Keratoconus and Cataract.
The Cataract surgery was made throughout the bare Descemet's membrane.

  • what was the point of doing the cataract surgery at the same time? It dramatically increased the chances of perforating Descemets and gained you nothing. You ended up doing a bad rhexis and had more trouble with the cataract than you otherwise would have. Additionally, you lose the opportunity to correct the astimatism you induce from the transplant with a toric lens later on. Just doesn't make any sense, unless the patient absolutely insisted on one surgery only.

  • This is quite the spectacle, more like a space mission than a surgery, especially that "iris-vacuuming" moment. What an ingenious procedure.
