I performed a Big Bubble Technique and a Phacoemulsification with IOL Implantation in a patient with Keratoconus and Cataract. The Cataract surgery was made throughout the bare Descemet's membrane.
what was the point of doing the cataract surgery at the same time? It dramatically increased the chances of perforating Descemets and gained you nothing. You ended up doing a bad rhexis and had more trouble with the cataract than you otherwise would have. Additionally, you lose the opportunity to correct the astimatism you induce from the transplant with a toric lens later on. Just doesn't make any sense, unless the patient absolutely insisted on one surgery only.
