The reason he used the president's middle name is simple. In private Republican undoubtedly refer to the president by his full name all the time so they keep themselves good and spooked over him. This goober made the mistake of saying in public what they normally only say in private.
There are lots of things Repubs believe that they can't say publicly. Birth control should be illegal, social secrurity and medicare should be destroyed, the minimum wage repealed, taxes increased on the poor.
