Crank up the difficulty and they will do it. You need to be at least one space away from them. You dodge it by taking a step back.

The skull bombs that the Ghoul tosses is the spell "Boom" - one of many new ones that the hero gets in QfG5.

You did better than I did against a ghoul on high, but I mostly played around medium.
