Mistakes and problem tell you that something is not being done correctly. Rather than blame the customer, mistakes need to be dealt with to resolve the customer issue. Good firms deal with them promptly, use the mistake as feedback to improve and give the issue authority and resources to allow it to be dealt with correctly.
