I'm sometimes seeing two notes on the pedals when there's just one note in the music. For example, at 1:27, the music has an A, and both the G-sharp and A pedals are pressed. Is this to get a special effect (in which case, is there a name for this technique)? Or just a mistake?

You are very observant about seeing the apparent G# and A pedals being played simultaneously. The G# corresponds to the regular E pedal -- the pedalboard "looks" like C then FGA. The E pedal is where G# usually is.
Cheers,
Joe
