maybe the reason why keiko was so confused about his sumimasen vs gomen-nasai thing is because he really didn't know what he's talking about.... which you'd hope he would. gomen/gomen-nasai is more informal, for instance if you're talking to a teacher you want to say sumimasenn. (at least according to my university japanese teachers which are from japan)
