As you probably know your FAA license never expire, but to remain current, it has to be validated every 24 calendar months by a Biannual Flight Review.
The BFR Biannual Flight Review need to be conducted by a FAA instructor CFI or a FAA examiner.
The BFR consists of a minimum one hour ground training, and an one hour flight.
During the BFR you must demonstrate that you are capable of performing the maneuvers and flight skills of your license PPL, CPL or ATP
To determine this, the PTS Practical Test Standards are used, just like when you did your check ride.
If you are not up to standards, the CFI will tailor a training program for you to improve whatever areas that need to improve.
