Here's a grid layout for a really simple teleport stage. Just choose the smallest stage and follow this:

The same coordinates work on the smaller stages - just count the number of blocks over and down.
The colored boxes represent the top block of the elevator. Blue boxes mean that you need to flip/mirror the elevator for it to work, and red boxes mean leave it alone. I strongly suggest you stick with the red boxes until you really know what you're doing. It's very easy to get confused if you're making a stage with both flipped and non-flipped elevators.
Remember, it will ONLY work in these positions, and you need to have some kind of sloped piece going down toward the highest point the elevator reaches. And sometimes having blocks touching the elevator other than the ones at the top will partially negate the glitch.

A small warning: Playing around with mirrored elevators will probably get very confusing, since you can't easily tell which ones are mirrored.
Here's a simple stage with 2 mirrored elevators and 2 normal. It's the same one from this video. Be prepared to get frustrated if you mess around with it too much though.

If it still doesn't work for you, first make sure your elevator isn't mirrored, then pay closer attention to the exact position of every piece, and then make sure you don't add any extras until you have gotten it to work. This does work. Just make sure you copy everything exactly.
Credit to AKALink227, bearsaremyfav, XeroHeroes, and possibly others for finding it before me. I just thought I'd explore it a little. This is what I found. Yes, this works with any character and any size slope, but only the smallest size elevator. And as far as I can tell, it works with ANYTHING that moves along the floor, including Pokemon, assist trophies, Ice Climbers' Ice Shot, Donkey Kong's Hand Slap blasts, and Kirby's Final Cutter blast. Things that bounce along the floor or do not travel directly on the floor, however, like Mario's Fireball, Pikachu's Thunder Jolt, or, strangely, Hotheads, will not be affected, at least not in the normal way.

@Smasher869 If the elevator's already in the right position for teleporting to work, just put some blocks next to (not above) the top part of the elevator.
