If you have a posterior polar cataract, you must use a special technique like I show here in order to maximize success and minimize complications.
Note the careful hydro-delineation and then the visco-dissection.
The capsular bag is kept inflated at all times and never allowed to collapse or come forwards.
The three piece IOL is placed with the haptics in the sulcus with the optic captured through the capsulorhexis. This allows for strong, stable fixation without risk of touching or damaging the fragile posterior capsule.

Beautiful surgery. However, I tend to put the IOL in the bag if the capsule is intact.
