According to Bolt Thrower's official site, cover design and booklet for Realms of Chaos were done by GW, who owns the rights to that artwork (not to the band's logo, though!). When Earache released remastered Realms of Chaos (BT already switched to Metal Blade then), GW forbade Earache to publish the original artwork and new artwork was done by former GW employee. Bolt Thrower never approved of publishing the new RoC, and urged the fans not to buy it.
