Colorado Culture Change Coalition

The Colorado Culture Change Coalition seeks to transform the culture of long-term care from an institutional model to a home model. Culture change encourages nursing homes and assisted living facilities to deliver care directed by the elders and value the input of front-line caregiving staff. Those who live and work in these settings deserve dignity and respect.


CCCC is a community of dedicated individuals, including Elders and others residing in homes, families, advocates, long-term care providers, long-term care ombudsmen, regulators, and health care professionals, committed to transforming the culture of the long term care continuum.
