This video shows a true polyp arising from middle ear mucosa. There is on-going turbid mucoid discharge and evidence of subtotal perforation. Blunt probing is usually carried out to ascertain its base or stalk if pedunculated type. Polypectomy by avulsion is to be avoided as it may be attached to the ossicle which can result in ossicular injury. Probing revealed the polyp is quite mobile indicating it has narrow pedicle most likely originating from malleus superiorly. Underlying chronic mastoiditis needs to be ruled out as the cause of persistent ear discharge. [ Green arrow- facial nerve, Blue interrupted circle- round window niche, Yellow arrow- probable attachment of the polyp, Oval blue- eustachian tube orifice, Dotted red- perforation margin]
This test is usually performed when the patient under GA just prior to definitive procedure; so it neither felt nor perceived as painful to the patient.
