Frontline Stories of Integrated Care in Colorado

A community mental health center and community health center in Boulder, Colorado work together to offer patients integrated care. This process of comprehensive care was part of a project with the National Council for Community Behavioral Healthcare. This video is part of an initiative by the National Center for Primary Care of Morehouse School of Medicine to recognize the importance of integrated care and best practices.
