First off, I didn't do it. The guys at N-Philes did.
They used a Gameshark to hack into RE4, and they found a secret model of Ada. It's her Mercenaries outfit with the skirt modded off, with 4 sets of 5 fingers, and an extremely buggy hit detection. When it gets hit, she starts twisting and the game plays continuous explosion and water splashing sounds.
