lock out tag out is a safety measure where you lock up the switch to equipment and keep the key so noone switches it on while you're working on it.

No blessings here. These guys are disabled for life. Three off work basically forever. Arc Flash incident. Two guys so badly burned they will likely never work again. The third guy was the supervisor and tramatized by the incident.

They Didn't lock out Tag Out while inspecting Engerized 1380 Cabinets. Plus they did not have the proper PPE on either.
