Ah yes. You have to right target the person as if you're picking their pocket and then just hit the X button. Instant kill. And it usually scares the devil out of the guards. They run screaming.

Amazing! How the hell did you do that??? I only knew a way to counter attack with hidden blade, which is block attack, grab the guy and throw him to the floor, then change to hidden blade and attack.

It is just the same as every counter. Just the point in time you have to press [Attack] differs from weapon to weapon. For the sword, [Attack] has to be pressed early, for the hidden blade quite late. The dagger is somewhere in between.
It is extremely important that your FPS are high enough. Below 20 FPS you'll propably fail every time.
