you're missing a step: the initial metabolism (heme --> biliverdin --> bilirubin) occurs in macrophages (the reticuloendothelial system), and only then is the bilirubin transported via albumin to the liver.

@babYDOLL908 as said, jaundice isn't a condition in itself, more a symptom of many possible conditions. Some conditions which cause jaundice are fatal, for example hepatitis and pancreatic cancer.
