According to Aaron in "The Brothers Neville" the autobiography they did with David Ritz; Aaron had gotten that tattoo in his teens, which sent his father, Arthur Neville Sr into a tizzy. He marched Aaron straight to the bathroom to try and scrub it off. Didn't work. Excellent book, please check it out if you have the chance.

i've always wondered about the story behind his cheek tattoo and was checking wikipedia today to see if an explanation was on there (there isn't one) and thought i'd check YT to see if he ever spoke about it - so, now I know. thanks for posting this!
