The reason why he does that is because to make it look like he had scars Heath Ledger had to wear mouth prosthetics and like anyone would do he developed a habit of feeling it with his tongue. Someone working on the movie noticed this and decided it would add something to the character.

ya wanna know how i got these scars? when i was a kid i scooped out some jolly joker's peanut butter substitute with a knife and decided to have a taste. then, somehow, the knife slipped. then there was peanut butter in more places than just the roof of my mouth...

He's always licking, because of the scars on the sides of his mouth.
Or the makeup was cherry flavored. Possibly Strawberry.
