I have to assume it was someone who does his own reloading and did not bother to consult his reloading tables, or was not checking the powder load after every tenth load for any changes. B.T.W. it is as dangerous to UNDERLOAD as it is overload a cartridge, due to pressure spikes created in the cartridge while in the cylinder.

or what happened to me, someone and ammo manufacturing put 2 soft lead bullets into one cartridge.

Not necessarily too much powder. More likely, a bullet was stuck in the barrel and when the next round was fired, the pressure spiked to beyond the revolver's ability to handle it.
Whenever you hear a strange or muffled sound when firing a weapon, always check to be sure the projectile in not stuck in the barrel. Better safe than sorry.
