This is on PolyphonicBranch's album "Heaven".

I tried to annotation-sub it, but it's out of sync. I'm not too good with this. Annotations must be on to see the subs.

EDIT EDIT: Guys, I was surfing pixiv and noticed BUZZ's (the artist's) upload for this image. She has "Forgotten Alice" on it, so it's pretty safe to assume that's the official English title.
