My friend and I made this as an engagement gift for another friend of ours.
Taking a little inspiration from 'raptorofaxys', we used a wireless doorbell that is fully customisable and wired all of the buttons on the controller (minus the D-Pad) to trigger the chime.
The original wireless doorbell was from Honeywell (http://yourhome.honeywell.com/home/Products/Door+Chimes/Wireless+Chimes/RCWL2205A.htm)
It comes with a USB dongle and can be customised with three sounds of your choice. The switch on the back of the NES controller changes the doorbell from cycling through all of the sounds in sequence to playing just one.

@raptorofaxys Thanks dude, the only thing that doesn't trigger the chime on ours is the D-Pad, we gutted everything behind it to fit the batteries so it's just fixed into place.
