The top half of the lander is a separate vehicle, when the mission is over it fires it's rockets, separates and docks with the orbiting command module. The astronauts transfer to the CM and the lander is jettisoned.

Videos and pictures taken, telemetry of equipment, spaceships, CM, SM, LM, LRV, H and J missions, rockets, SIVB and SaturnV, the astronauts that went there, sixty scientific experiments left on the surface and the data received from them, the 840 pounds of rocks and six foot long intact core tube samples brought back, satellites launched from the SIM bays into lunar orbit, reflectors left at the landing sites, etc. etc. etc. etc.

The evidence is in the public domain for decades. The technical data has been available for reviews for decades, some of it in advance of the landings. Planned television coverage, thousands of photographs released, hours of video and audio. Think back to the 60's and the movies that were made then. Could NASA have produced such fakery that it would not only stand up to the scrutiny of a 1969 audience, but also several generations of engineers and scientists?
Alas no, but either one of the main Apollo add-ons for Orbiter do in surprising detail.

Is this a playback of the actual lander state (position, velocity, attitude vs time), or is it your own piloting?

The forward motion from P64 is also a good approximation of how Armstrong brought it in. I don't think his final descent was nearly as vertical though. The ideal flight profile would have had the LM come in completely vertically from the last one hundred feet. Given the circumstances though, Armstrong claimed in the debriefings that he wasn't flying the LM very well at this point but, given the circumstances, it was clearly still absolutely outstanding.
