It can be also from the bad engine oil , slow water pump , locked radiator grill , low round/minute radiator fan , scales in piping system if you are using tape water , timing belt goes wrong , and many more.....

not true not always, the more efficiant the engine runs the less power is wasted to heat

I think that retared timing makes it run cooler not hotter.
