What did he do in the book he did not in the film?

He bought prepubescent boy slaves and had sex with them on a regular basis. He also forced his nephew Feyd to personally kill all the slaves he slept with for having tried to assassinate him (by implanting a needle into one of his prepubescent boy slaves unwittingly) while he watched. And he sacrificed Rabban to the Fremen. In one of the other books he ruined somebody's love life by manipulating her into sleeping with the men he liked so he could vicariously sleep with them (long long story).

You're right they weren't prepubescent, but they were underage. Feyd actually surgically implanted a small needle filled with poison into the kid's inner thigh just under the surface of the boy's skin so when the Baron grabbed his leg to bring him in, he'd stab himself in the hand. It sounds way too elaborate, but in the context of the Duniverse it's commonplace. Don't ask me why the poison didn't kill the boy instantly, but I think that was one of the complications that lead to the boy's death.

Yes, I got that the struggle between Leto and Hark had undertones to the cold war. Also the heart plugs were Lynch's idea they were not in Herbert's book. Also in the book the Baron fucked boys but he didn't kill them.
